Audit Scores
In one example, we faced a challenge where a safety management system was experiencing consistently low internal and external audit scores. After working with operations management to fix a few key areas, we were able to see a consistent internal audit score of more than 96% and an external certification audit score of more than 98%.
Incident Reduction Strategy
During a period of increased work volume a group of workers experienced a high volume of both recordable and non-recordable personnel incidents. We workerd with site management and company leaders to identify the immediate and root causes for the majorirty of the incidents that were taking place. After implementing the appropriate corrective actions, the group dramatically reduced their recordable incident rate. In this particular instance, they experienced zero recordable incidents in more than 830 days. For this group, this time frame totalled more than 304,000 working hours.
Documentation Improvements
One group of workers frequently used potentially hazardous chemicals in their workplace. If the protocols listed in the material safety data sheet information was not followed, a serious incident was very likely to occur. Due to the highly repetitive nature of the work being done, the employer noticed that some workers were becoming complacent in their reviews of the product safety information. A review of the safety documentation system revealed an opportunity to create an easy to use checklist for the workers and improve the company's safety program.
Hazard Identification Campaign
After conducting an examination of the type and nature of equipment and personnel injuries that were being reported, we were able to develop a program that educated a group of workers on the types of hazards that were most likely to cause an incident in their work environment. Over time, equipment and personal injury incidents decreased significantly and both hazard identification cards and hazard assessment exercises improved.
